Information required by law
Please describe the case and focus on:
  • Whether the case describes improvements, unethical or illegal conduct.
  • Names of relevant departments or job positions.
  • How the case developed, mainly what, when and where.
  • Please do not provide personal data of the persons concerned. We will make them illegible to protect privacy and personal data.

You can choose several files, each up to 8 MB.

Under Section 12(2)(a) of the Czech Whistleblower Protection Act we will then not confirm receipt. You still get the filing code to add information and learn the outcome.

After sending you will see the unique number of your filing. With it you can check the status, add information and learn the outcome. Management receives only summary overviews without any data about you.

Information required by law

This form is the internal reporting system of VZOR/SAMPLE under Czech Act No. 171/2023 Coll., on the protection of whistleblowers. The system is operated by the law firm Haládik & Partners.

Reports are read only by the competent person Mgr. Jozef Haládik, advokát, Haládik & Partners, advokátní kancelář, spol. s r.o., e-mail oznameni@ethicsline.eu.

You can submit a report in writing through this form or by e-mail to oznameni@ethicsline.eu, orally, or in person; we will arrange a meeting within 14 days of your request.

We will confirm receipt within 7 days and inform you of the outcome within 30 days (at most 90 days in complex cases).

You can also report to the Ministry of Justice of the Czech Republic (oznamovatel.justice.cz).

If you give your name and contact details in the report, we will not disclose your identity to anyone without your written consent. We also accept and assess anonymous reports, but they are not reports under the Act and the statutory time limits do not apply to them.

We also accept reports from persons who do not work for the company (for example job applicants or suppliers).
